Symptom narrative

Systemic: Adult dose and pediatric dose given-Mild, Additional Details: I was administering 2 vaccines to each member of the family (family of 4). When patient sat down I went too fast and gave her an adult covid shot (info above), then I gave her the pediatric covid shot. I thought that I was giving her the flu shot and the pediatric covid shot. I informed the family and told them to monitor for potential adverse reactions. Will follow up Monday.
