Symptom narrative

Site: Bruising at Injection Site-Medium, Site: Itching at Injection Site-Medium, Site: Pain at Injection Site-Medium, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Medium, Additional Details: Patient received the 2nd dose of Shingrix on 11/2/25; administered by Rph on duty; Patient presented to pharmacy 11/3/25 to speak with PIC; Patient presented a picture showing the back of the left arm red and swollen; Patient stated that when she questioned the site of administration, RPh stated that it was to be given subcutaneously, and previous administration was done incorrectly; However, PIC showed patient on the box where it states to be given "intramuscularly only".
