Symptom narrative

Site: Itching at Injection Site-Mild, Site: Pain at Injection Site-Medium, Site: Redness at Injection Site-Medium, Additional Details: Patient came in on 11/4/2025 wanting the second dose for Shingrix. That was when she informed me she developed a rash that was about 7 inches down her shoulder. I contact her PCP and doctor suggested not to adminster the second dose.
