Symptom narrative

The patient stated she started having a headache, pain in her neck, pain, numbness and tingling in her left arm all the way down to her fingers as well as on and around her left scapula. I asked if she noticed anything when she got the shot and she said no. She did state this started approximately 2 hours after she got the shot. She told me she went to the urgent care and was given steroids and is still taking steroids. She said she has trouble moving or lifting her left arm. She also mentioned that she was to start attending physical therapy, as it still hurt and seemed to only be slightly improving. She then said they mentioned if it didn't get better they may have to do surgery, which I wasn't sure I understood since she had only been to urgent care.
