patient states day after receiving shot started with itching and puffiness in face and forehead. this progressed to move down the body. she states had overall rash and severe itching. she has been taking benadryl and using cortisone cream otc.

insomnia, hypertension, hypercholesterolemia

zolpidem 5mg, atenolol 25mg , pravastatin 20mg, lisinopril/hctz 20/12.5, escitalopram 20mg
