Symptom narrative

Vaccine was administered. Around 7 pm, pt started experiencing symptoms. Redness at the injection sight. Swelling over her face, neck, ears and armpit. No swelling in the mouth, but rash all over her face, instead her mouth. Also had a headache the following morning. No treatment was given.

Medical history

asthma, HPV, Squamous lesion intraepithelial cervix, anxiety, GERD, HTN, atherosclerotic heart disease

Other medications

albuterol, ammonium lactate, aspirin, cholecalciferol, cyanocobalamin, ibuprofen, levonorgestrel, lisinopril, multivitamin, nitroglycerin, omeprazole, rosuvastatin, Wegovy, sertraline, trazodone.
