Symptom narrative

patient was administered with a shot of Boostrix instead of the scheduled Vaxelis; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong vaccine administered in a 3-year-old male patient who received DTPa (Reduced antigen) (Boostrix) (batch number 9JT4S, expiry date 21-FEB-2027) for prophylaxis. Co-suspect products included VAXELIS for prophylaxis. Previously administered products included VAXELIS (Last dose of Vaxelis was administered back on 21-FEB-2023). On 29-OCT-2025, the patient received the 1st dose of Boostrix. The patient did not receive VAXELIS. On 29-OCT-2025, an unknown time after receiving Boostrix, the patient experienced wrong vaccine administered (Verbatim: patient was administered with a shot of Boostrix instead of the scheduled Vaxelis). The outcome of the wrong vaccine administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 06-NOV-2025 A 3-year-old patient was administered with a shot of Boostrix instead of the scheduled Vaxelis. Last dose of Vaxelis was administered back on 21-FEB-2023 and it was a third dose. She called to get guidance after the incident and to know if there were some safety concerns.
