Symptom narrative

Patient received Arexvy 9/12/24 at another pharmacy. Patient came into pharmacy 11/6/25 requesting an RSV vaccine at which time it was administered. There are notes the registry was checked and did not show history of already receiving the vaccine. Pharmacy system failed to alert of this dose as duplicated. Patient was informed after internal audit was conducted. She has not experienced any negative side effects and reported she was unaware she even received a dose in 2024.
