Symptom narrative

On 11/10, pt presented with excoriation approx 1.5 in long by 1 in wide over site. Pt states swelling had begun about 30 min after injection. States site is much improved. Advised PT to use topical abc and self adhesive bandage to keep site free from infection, inform PCP of reaction, and get seen immediately should sx recur or get worse. Pt states understanding and desire to comply .
