Symptom narrative

Patient received second dose of Arexvy in error. First dose was given 11/4/24 and second dose 11/11/25. We realized after the fact. Patient was made aware of the error and told to monitor and report any adverse effects. Provider was also notified and vaccine was documented as it should be.
