Patient within 5 minutes of receiving Varicella vaccine started having system itching. No other symptoms. Benadryl was given. Patient upon reassessment a few minutes later was noted to have new diminished left lung sounds and pulse ox had decreased to 90%. Patient was given 0.3 mg epi pen. Few minutes after receiving epi pen patient had 2 episodes of emesis. 911 was called to send patient to ED for further observation. Before patient left with EMS in ambulance, patients no longer had diminished lung sounds on the left and pulse ox had improved to 97% and itching had resolved. BP remained stable through entire observation period. HR remained elevated at 130.
Mild cold like symptoms, with no fevers. Activity was at baseline. Physical exam unremarkable except nasal congestion.

Patient is on claritin for seasonal allergies.

Patient is from (state withheld) and received vaccines at Medical Center. Our clinic is attempted to obtain vaccine records but we were noted father had to sign their HIPPA form either thru online database or in person at their clinic. Father was unable to do it through online database and wanted to vaccinate patient for school. Serology testing for Varicella was done, which showed patient was not immune to Varicella, and thus Varicella vaccine was given.
