Symptom narrative

MA went to draw up vaccines for a 2-month-old baby. He was due for Pediarix, Hib PCV-15, and RoTA. I went to grab the vaccines out of the refrigerator, usually on the top shelf, and not looking at the boxes, cause they are usually correct, she grabbed what she thought was the right vaccines. Pediarix, Hib, and Rocephin were all in the right bins, and Gardasil 9 was in the bin labeled PCV15. Trusting my coworkers, I grabbed a Gardasil vaccine, not thinking it wasn't PCV15. She gave the baby Gardasil in his right leg at the time of his visit.
