Wrong vaccine administered; This non-serious case was reported by a physician via call center representative and described the occurrence of wrong vaccine administered in a 15-month-old male patient who received DTPa-HBV-IPV (Pediarix) (batch number 4F9T5, expiry date 01-MAR-2027) for prophylaxis. Co-suspect products included DTPa (Infanrix) for prophylaxis. Previously administered products included Polio vaccine (received Polio vaccine on an unknown date) and Hepatitis B vaccine (received Hepatitis B vaccine on an unknown date). On 30-JUL-2025, the patient received Pediarix. The patient did not receive Infanrix. On 30-JUL-2025, an unknown time after receiving Pediarix, the patient experienced wrong vaccine administered (Verbatim: Wrong vaccine administered). Medical Doctor/Pediatrician wanted some guidance on this topic. The patient received previously another Hepatitis B, Polio vaccine (no brand name was specified from the Medical Doctor/Pediatrician´s end). Vaccination date, vaccine detail and patient demographics were obtained in this call.
