just the diluent of the Shingrix vaccine without mixing in the adjuvant.; just the diluent of the Shingrix vaccine without mixing in the adjuvant.; This non-serious case was reported by a other health professional via sales rep and described the occurrence of inappropriate dose of vaccine administered in a patient who received Herpes zoster (Shingrix) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced inappropriate dose of vaccine administered (Verbatim: just the diluent of the Shingrix vaccine without mixing in the adjuvant.) and inappropriate preparation of medication (Verbatim: just the diluent of the Shingrix vaccine without mixing in the adjuvant.). The outcome of the inappropriate dose of vaccine administered and inappropriate preparation of medication were not applicable. Additional Information: GSK Receipt Date: 07-NOV-2025 The health care professional reported that the nurse gave just the diluent of the Shingrix vaccine without mixing in the adjuvant. The patient was then given the proper reconstituted vaccine and asked if she could get one vial of the diluent. The reporter did not have information on hand about patient. Representative called vaccine service center to report this to medical information as well as ask them if she was able to get one vial. The batch number was not provided upon follow up with the reporter.
