Symptom narrative

Pt. states injection site was red and swollen immediately after dose; soreness and weakness in left arm in following days persisting until more than 30 days later. Pt. also states that he had a severe eczematous reaction in the weeks after the vaccination. Areas included: bilateral arms/hands. Unsure if pt. has a history of eczema/psoriasis prior to receiving vaccine.
