Symptom narrative

She is weak, achy, feeling unwell. Daughter reports patient can't think of words, or put sentences together Forgetful of her surroundings. Very groggy and just wants to sleep. This has been going on for 2.5 weeks

Unstable gait, macrocytic anemia, hyperglycemia, history of embolic stroke, urinary frequency, overactive bladder, recurrent falls, thrombocytopenia, Epilepsy,

Other medications

Acetaminophen 325mg every 6 hours PRN pain, ascorbic acid 500mg chewable 1 tablet daily, Asprin 81mg daily, atorvastatin 40mg 1 tablet nightly, biotin 5000 mcg cap two times a day, cyancobalamin 1,000 mcg/ml 1ml IM every 30 days, levetirace

Amoxicillin, Codeine, Morphine, Terramycin
