From patient's chart on morning of 10/29, per Triage RN: "Per mom red itchy rash appeared on bilateral thighs starting that evening of 10/27. Mom stated the rash became worse, spreading across thighs, but is now improving today. Mom did not given any otc medications for the rash or symptoms. Mom would like this reported and would like to know how to proceed for further flu vaccinations. "
