Symptom narrative

Persistent pain and weakness of the left shoulder for greater than four weeks. Patient reports vaccine administered in the left shoulder joint space as opposed to left deltoid. Seen by Employee Health and referred to orthopedic surgery shoulder specialist for further evaluation. No imaging available at the time of event reporting, but physical exam consistent with neurologic or left shoulder musculoskeletal pathology possibly related to vaccine injection.
