Symptom narrative

Patient passed out in restroom about 5 minutes after sitting after immunization. He fainted and hit his head on the floor of the restroom. RN checked his blood pressure and Dr. checked the back of his head. There were no cuts on his head, just sore to the touch. He sat for about 15 more minutes before getting up. Dr. examined him before allowing him to get up.
