Symptom narrative

After double-verification of necessary immunizations with second RN, this RN gave the following immunizations at patient's 4mo WCC: Prevnar 20, ActHIB, RotaTeq, and Kinrix. Vaccine coordinator notified RN this AM that Kinrix cannot be given to any person under 4 years of age per CDC recommendations; this adverse event was found to have occurred during routine paperwork review of the screening checklist for the vaccines from that appointment (10/2/25).
