Symptom narrative

During vaccination, patient was sitting upright in chair. Nurse administered vaccines and while administering, patient slumped over and then tightened-body and shook upper extremities for 1-2 seconds. BP 100/62, BS 118, RR16. Normal cardiac and neuro exam, A&O x 3 and acting per usual. No significant fatigue but had pt drink an apple juice and lay down on exam table. Patient reports that he has drank water today and at a chicken burrito for lunch.
