Product reconstitution error; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate reconstitution technique in a patient who received Herpes zoster (Shingrix) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced inappropriate reconstitution technique (Verbatim: Product reconstitution error). The outcome of the inappropriate reconstitution technique was not applicable. Additional Information: GSK Receipt Date: 10-NOV-2025 The health care professional states mixing error when reconstituting Shingrix vaccine. No further information was obtained. The batch number was not provided upon follow up with the reporter.
