Patient's wife made appt online for an immunization that was supposed to be Covid. Upon arrival the pt was not sure what shot he was supposed to get, he said "whatever the dr office sent is what I am supposed to get." Looking in his file the dr office sent on 11/4 rxs for shingrix and hiberix. So i asked him about shingrix and he said ok. We spoke extensively before and after his shot about it being a shingles shot. After he left, his wife came in and said that the shot was supposed to be a covid shot not a shingles shot. His wife said she gave him a note to give to us that said for him to get a covid shot, but he did not give us this note. Speaking to his wife, it turns out he received Shingrix, PNV, HIV all at a drs appt on 11/42/25. I do not know why they administered a shingrix shot on 11/4 AND sent us a rx for him to get the shot. So he received 2 shingrix vaccines just 7 days apart. I called the dr office 2 separate times and spoke to 2 different employees and both put in a message for the office to call us back asap but we still have not heard back. As of 6pm the day of vaccine, pt was feeling fine with no issues at all.
