Symptom narrative

No other AE/No PQC; first dose of GARDASIL 9 on 26JUL2025 but did not come in at the 2 month mark to receive his second dose (should have been received 26SEP2025). His second dose will be given today 30OCT2025; This spontaneous report was received from pharmacist and refers to 39-year-old male patient. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 26-Jul-2025, the patient started therapy with Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL9) dose number 1, 0.5 mL, as prophylaxis (lot # and expiration date were not reported). The patient started therapy with Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL®9), for the treatment of prophylaxis. The patient did not come in at the 2 month mark to receive his second dose (should have been received 26-SEP-2025). His second dose will be given today 30-OCT-2025 instead (inappropriate schedule of product administration). No other adverse event (AE)/No product quality complaint (PQC).
