Symptom narrative

Patient presented to ED with burns to left fingers, right leg, abdomen/ trunk, and ear after falling in a fire pit on 11/10/25. Per incident report, vaccine was given intravenous route, rather than intended intramuscular route. Prefilled syringe was reportedly mistaken for hydromorphone IV syringe that was resting in the same place as the vaccine.

Current illness

Burn wounds to face, bilateral hands, and right side of body sustained on 11/10/25

Medical history

Type II Diabetes mellitus, chronic wound to right foot

Other medications

Home medications include amlodipine- benazepril 5-40 mg, levetiracetam 500 mg, rosuvastatin 10 mg , ozempic pen

Allergies

Sulfamethoxazole-trimethoprim (Mild rash)
