Symptom narrative

Patient did not wait 15 minutes after receiving the vaccines, patient got up and went to leave the office .Vasovagal syncope- 2/2 vaccines, no breakfast, hot room and heavy sweatshirt Patient received HPV and flu vaccines. Upon leaving office, she became dizzy and passed out for a few seconds. She hit her face on the wall, and suffered lip laceration and nose pain. Lip laceration on upper lip, oral mucosa side. Spontaneous hemostasis <5 min. No nose injuries or epistaxis. Gave patient water and candy. Did not eat breakfast prior to appointment and exam room was hot. Dizziness resolved, able to walk. Repeat vital signs WNL. Discussed standing and walking slowly today, good hydration. Would recommend not horse back riding today.
