Symptom narrative

Patient reported of blurry vision in few minutes of receiving vaccination followed by burning and tingling sensation of the tongue. patient also reported of swelling of throat to paramedics, he was later given some steroids, they observed him for 4 hours at the hospital and then later got discharged.
