Symptom narrative

No adverse effects were reported.; Pharmacist called to report that a dose of VARIVAX was administered while it was inadvertently being stored out of range. Pharmacist stated that it may have been being stored improperly as early as March 2025 and it was just discovered this week; This spontaneous report was received from a pharmacist and refers to a 1-year-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On early March 2025, a dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) injection was inadvertently stored out of range. It was discovered on 04-NOV-2025 (also reported as "this week"), when the patient was inadvertently vaccinated with it, 0.5 mL, lot #Y015100 [which has been verified to be valid lot number for Varicella Virus Vaccine Live (Oka-Merck)], expiration date reported and upon internal validation establish as 06-SEP-2026, administered for prophylaxis (strength, route and anatomical site of administration were not provided). The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported) (Product storage error). No adverse effects were reported.
