Symptom narrative

Patient stated she has no issues with vaccinations or any allergies. Was asked multiple times by RPH. After receiving vaccine patient fainted. RPH monitored patient blood pressure was checked multiple times was getting no improvement 130/50 110/50. RPH called 911. Patient stated after that she has history of fainting and low blood pressure but she did not mention this to RPH before vaccination. Also mentioned that she had egg allergy but doctor tested her and cleared her stating she has no allergy to eggs. 911 checked the patient and she refused hospitalization. RPH follow up with patient at 7PM 11/3 she stated she is doing ok. RPH recommended observation and follow up with PCP.
