Student vaccinated at mass school clinic on 10/18/25. Vaccine documented given Subcutaneous, but should have been given Intramuscular. No adverse reactions reported at time of event. Left message for parent on 11/14/25 of vaccine error. Unknown at this time if there were any adverse reactions. Vaccine dose is valid and does not need to be repeated.
