Patient states received flu vaccine on 10-6-25 from our office. We do not have any documentation that we administered a vaccine to her. Listed vaccine as unknown because we cannot confirm what she received. Arm pain- says got flu shot on 10/6 says symptoms on rt arm; Says pain radiates to the back of the neck Says has a headache because of the neck pain ; Says took tylenol but symptoms Says no swelling on the fingers Says has redness on the injection site and has swelling as well Office visit on 11-11-25: Acute pain of right shoulder - Acute, not at goal. - Pt with acute right shoulder pain following flu vaccine 1 month ago. - Suspect tendinitis. Recommended PT, patient declined, plans to do exercises at home. - Discussed differential including possible component of cervical radiculopathy. Could consider EMG if symptoms are persistent after 1 month.
