Symptom narrative

Patient received the Kinrix vaccine that is meant for age group 4-6 yrs. Instead of the patient receiving the Vaxelis vaccine got his age group. According to his vaccine database he was not due for a Hep B vaccine and the other was given with out realizing that the age group difference.
