Symptom narrative

Client was sitting in chair in clinic room with employee sitting at his right side, vaccine administration had just been completed. Four vaccines were administered in the following order: IPV, HPV9, Hepatitis B and Tdap. All four vaccines were a third and final dose in each series. Client health history intake was negative with the exception of being anxious about receiving vaccines. Client was given instruction on anxiety-relieving measures prior to vaccine administration and employee talked with client throughout process. Client was alert and oriented, talking with employee, then client's eyes rolled to back of head, eyes closed and he fainted, body starting to go to the L side. Employee was able to catch and hold client upright in his chair, as to avoid any head injury or fall, simultaneously called out for help. Client was diaphoretic during and after syncopal episode. Nurse Practioner, employee's supervisor and another PHN came into clinic room with employee and client. Cool compress applied to his face and neck while ice pack was obtained and given water to sip. Client complained of dizziness and he asked to lay down on exam table. He was carefully assisted by staff to exam table in semi-Fowler's position, ice pack placed to back of neck and vital signs obtained. Initial vital signs at 1:52pm: BP 89/52 HR 54. Never disoriented, able to correctly count number of fingers employee was holding up, denied double vision. He was asked if he had eaten or drank anything prior to appointment and he replied with "not much," as he had gotten distracted through the day. Apple juice and pretzels were offered, slowly eaten and sipped. No nausea or vomiting. Diaphoresis resolved. Repeat vital signs at 1:56pm: BP 103/65 HR 55. Client stated he felt better, dizziness had resolved and that he was ready to get up. Employee requested client to remain settled in place for several more minutes and allow for recovery. Employee asked client was asked if employee could contact a family member or friend; he stated his cousin was waiting in the visitor parking lot for him. Client called his cousin and he came in to clinic room. With client's verbal permission, cousin was updated on event. Vital signs at 2:02pm: BP 115/73 HR 70. With employee standing next to client, client was directed to slowly transition from laying to sitting to standing and then ambulating. Client independently able to do so, without any side effects or complaints. Client and cousin were both given post clinic instructions; both client and his cousin verbalized understanding and thanked staff for assistance. Client was called by employee on 11/14/2025 at 2:30pm to check in on him after immunization clinic yesterday, 11/13/2025. Client stated that his injection sites (arms) were sore and he had had a fever yesterday (he did not know the specific temperature), but that he was feeling better today. He has rested and is eating and drinking. He denied taking any over the counter medications such as acetaminophen or ibuprofen. employee again suggested some post-vaccination interventions, such as cool compresses to injection sites, over-the-counter antipyretics/analgesics and to continue staying hydrated. For any severe side effects he needs to call EMS but if he had any general questions, he could call the employee and contact number was given.
