Symptom narrative

mother returned to clinic with patient about 20 minutes from leaving the clinic. states patient had been crying after immunizations, and she had noticed his face red and swollen initially she thought it was from him crying but his right eye was getting so swollen that it was closing up. Arrived at clinic and provider evaluated patient and noted facial swelling, wheezing and hives starting on extremities. Patient was given oral Benadryl 25mg and oral prednisone 10ml and albuterol breathing treatment. EMS was contacted and patient left the clinic awake and oriented, VS stable.
