Did not mix Shingrix solutions; gave patient diluted solution before mixing the product; This non-serious case was reported by a other health professional via sales rep and described the occurrence of inappropriate preparation of medication in a elderly patient who received Herpes zoster (Shingrix) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Did not mix Shingrix solutions) and inappropriate dose of vaccine administered (Verbatim: gave patient diluted solution before mixing the product). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 07-NOV-2025 Nurse gave patient diluted solution before mixing the product. Nurse noticed mistake and gave patient another vaccine mixed correctly. The batch number was not provided, and unable to contact the reporter.
