11/03/25 Patient's mother, came to pharmacy with concerns of adult son getting 2 immunizations and patient's mother believes she did not see pharmacist take cap off of syringe when giving second immunization on right side. Patient's mother believes cap was left off on the table when pharmacist moved to the other side. Spoke with pharmacist that gave immunization and they stated that did not happen. Patient's mother states that this is a lie but voiced concerns but did not want pharmacy to contact son (patient) in any way concerning this matter and with no indication of patient of feeling ill on unwell when asked if they were feeling okay. Patient and mother came to pharmacy 11/15/25 at 12:26 pm to pick up written documentation of requested concerns addressed. Patient then directly stated themselves that they have been "feeling sharp pain since day of immunization (COVID) on 10/17/25 and everyday since in right arm. When moving the right arm can move for a few minutes but cannot move much for long." Patient states left arm is feeling okay and no other symptoms. Contacted physician with message and will re-contact when in on Monday.
