Symptom narrative

Patient reported having chills the evening and night after the vaccination. The following morning, the patient passed out in the bathroom and hit her ribs on the bathtub and hit her head on the floor. Patient was unable to stand on her own upon coming to, and was escorted via EMS to the hospital. Patient reports no longer having any issues beyond bruising.
