Pt presented to pharmacy with family member with appointments to receive Covid vaccination. Pt had previously received current Covid vaccine in September 2025, but did not remember this; potential duplication of vaccine was caught by neither pharmacy staff nor pharmacy computer system. Pt received non-recommended dose of Comirnaty at that appointment. Per in-person check-in with pt 5 days following event, pt is not experiencing any adverse signs/symptoms related to administration error.
