Symptom narrative

Patient developed acute onset of severe neck pain about 5 days after injection. Developed symptoms around 12:30 while at work on 4/2/2025. Went to UC and was given Toradol 30 mg IM. No pain control, so daughter drove her to the ED. In the ED diagnosis of neck spasm. In ED, they obtained a CT angio of the head and neck which was completely normal. She has a h/o xray of neck 8/23/2017 w/ "Straightening of the normal cervical lordosis with 2 mm C5 on C6 retrolisthesis. Vertebral body heights are maintained. There is mild disc height loss at C5-6 with anterior osteophytes. Remaining disc spaces are preserved. No evidence of instability on the flexion-extension views. No fracture. Prevertebral soft tissue is normal." Meds given in ED include tizanidine 4 mg, lidocaine 4% patch, morphine sulfate 5 mg. She waws discharged w/ tizanidine 4 mg (20 tabs) and oxycodone 5 mg (10 tabs).

Degenerative disc disease of the cervical spine with retrolisthesis of C5 on C6, Shingles of forehead 9/2016, Hyperlipidemia, lichen sclerosis, TMJ syndrome, tobacco use disorder, endometriosis, chronic headache, chronic constipation

Lab data

CBC and BMP 4/2/25 at 17:18 all normal except chloride of 111 (normal of 98-107). CT angio of neck and head also same day with results as noted above, completely normal.
