No adverse event; Nurse reported administration of improperly stored PROQUAD on 11/7/2025. administered on 11/7/2025 to a single patient. No side effects reported. No additional AE/No PQC. ? Names of vaccines involved (including lot/exp) PROQUAD Z008223 10; This spontaneous report was received from a registered nurse concerning a patient of unknown age and gender. The patient's pertinent medical history, past drug reactions or allergies, concurrent conditions and concomitant therapies were not provided. On 07-NOV-2025, the patient was vaccinated with an improperly stored dose of measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (lot #Z008223, expiration date: 27-OCT-2026). The vaccine was reconstituted with sterile diluent (MERCK STERILE DILUENT) (lot number and expiration date not reported). No side effects reported. It was reported that temperature/time -14 degrees Celsius (°C) up to -10°C 0 hours 7 minutes 0 seconds; -10°C up to 8°C (0.10°C) 2 hours 7 minutes 0 seconds. Previous temperature excursion? NA
