Symptom narrative

incomplete dose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of incomplete dose administered in a 60-year-old female patient who received Herpes zoster (Shingrix) for prophylaxis. On 09-NOV-2025, the patient received Shingrix. On 09-NOV-2025, an unknown time after receiving Shingrix, the patient experienced incomplete dose administered (Verbatim: incomplete dose). The outcome of the incomplete dose administered was not applicable. Additional Information: GSK Receipt Date: 13-NOV-2025 A Pharmacist wanted to get recommendations after a patient received a dose of Shingrix, but part of the volume leaked out through the needle during the administration. The batch number was not provided upon follow- up with the reporter.
