Symptom narrative

Patient still has a very sore arm, 3 and a half weeks after receiving her injection. It is very painful despite icing and using pain relievers. She does not have restrictions in range of motion, nor does she have any numbness/tingling. Patient says the injection site is still a little black/blue, but the injection site never appeared abnormal (no bubbling/pooling of blood directly under the skin). Unfortunately, the patient's insurance has recently changed and she no longer has a primary care provider. She is going to consider going to an urgent care to be evaluated.

Other medications

Medroxyprogesterone 150mg/ml PFS (inject 1ml IM every 10 weeks), Venlafaxine 37.5mg (1qd with food), Hydroxyzine 25mg tabs (1tid prn)
