Symptom narrative

I had/have an ongoing allergic reaction and infection due to the Tdap vaccination, as well as a severe left shoulder injury. The nurse injected the vaccine without following proper intramuscular injection protocol which likely caused injury to the shoulder. He pulled my shirt collar and bra strap down over the shoulder instead of rolling up the sleeve, twice. I also believe he used the same hand to try to inject the shots and hold back the clothing at the same time. On the first attempt, the nurse lost control of my shirt collar and bra strap which caused the needle to fling out of the skin and upward. As he pulled down my clothing again, I offered to hold down the shirt and strap while he administered the vaccine. He declined my offer. He did not change the needle, nor do I recall him re-sterilizing the area, before his second attempt. He did inject the vaccine on the second try into the upper left shoulder. Within hours and/or days, the shot caused severe burning pain and inflammation in, not only my left shoulder, but also radiating pain and neuropathy throughout the entire left upper extremity (LUE), shoulder, neck, and head. Signs and symptoms include severe burning pain, inflammation, numbness, tingling, pins and needles, electric worm-like sensations, prickling, muscle aches and weakness, sensitivity to touch, and other related issues, especially in my left arm, hand, fingers and face. My left hand and fingers feeling like I am wearing an x-small glove for a xx-large hand. I am experiencing extremely high blood pressure due to significant pain. Severe redness (rash) and bruising developed over the injection site(s). I also developed two large, extremely painful swollen areas of soft tissue with harden knots at their tops. I have severe pain and blurry vision in my left eye, as well as redness, swelling, excessive tearing, and fatigue. My neck and side of face and jaw is extremely stiff; and, I have reduced ROM in my LUE. My head pain and sensitivity to light have worsened. I am having difficulty hearing. My left thumb makes irregular, jerky movements. I had rapid weight gain in a few weeks. And, I have an overwhelming sense of feeling unwell and severely fatigued. Allergic reactions include, but are not limited to, difficulty breathing, difficulty swallowing, feeling like a knee is pressing down on my chest, difficulty drawing a full breath, fast heartbeat, swelling, hives, rash, flushing, burning, dizziness, loss of appetite, severe abdominal pain, feel faint, feel feverish, chills, diarrhea, nausea, and vomiting. I developed cellulitis in the upper left arm.
