Symptom narrative

new patient from out of state, old Vaccine record was obtained and entered into the system. MA accidently added Meningococcal off the old record into the new record (fixed on 11/18/25) but also had a note from school stating that the child was not able to return to school unless his Vaccines were caught up. MA is certain that Meningococcal was on the letter and we are requesting a copy. Child was still 10 at the time of immunization
