Approximately 30 seconds after receiving vaccines, client asked for water and said she felt dizzy. Client leaned back in the chair and became pale and lost consciousness for approximately 5 seconds. Client did not fall out of chair, breathing spontaneously at this time. Client then made eye contact with nurse and sat up, began to cry and apologize, breathing quickly initially. RN stayed with nurse giving verbal reassurance, applying cool water bottle to back of neck, client slowed her breathing and then began to take sips of water. Color returned to client's face, client began smiling and conversing appropriately. Stated she has a history of fainting with blood draws and sometimes gets lightheaded with vaccines. Client BP 97/71 HR 58 SpO2 97% on RA. Client remained in office with RN for 15 minutes afterwards recovering, walked with steady gait independently, speaking clearly, and smiling apologizing.
