An expired vaccine was given to patient on 10/02/25 by employee. I notified the manufactures who said that they do not have data showing that it would be harmful. However, they do not recommend giving it past the expiration date. They said that a new DTap (Daptacel) would need to be given since this was not a valid dose. I also notified the provider who informed me that this would not be harmful since it was stored properly and so close to the expiration date and they will need to get the shot again. I also called the parent and left a voicemail and will continue to contact. I submitted a VEARS and internal Incident Report, reporting this situation and have had serious talks with staff.
