Patient was in office date of service 11/11/2025 for a follow up visit and received remaining vaccines DTAP / IPV "POLIO" on the left deltoid with no symptoms or reactions at given time or late evening. Mother states she noticed redness and warm to touch the next day 11/12/2025 around 8:57pm with no other symptoms besides a fever of 103, mom had gave Tylenol and Motrin PRN as needed. Patient was scheduled as a sick visit in office next day 11/13/ 2025 DR. advised mom to monitor size and alternate between Ibuprofen and Benadryl. Clindamycin was also prescribed x 7 days and started 11/14/2025. By Saturday 11/15/2025 it was still expanded in size with no other symptoms and cleared up by Sunday 11/16/2025 I spoke to mother 11/17/2025 patient was doing much better and has recovered.
