Symptom narrative

Maladministration of the adjuvant only; Maladministration of the adjuvant only; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 62-year-old female patient who received Herpes zoster (Shingrix) (batch number NM35M, expiry date 06-JUN-2027) for prophylaxis. On 17-OCT-2025, the patient received the 1st dose of Shingrix. On 17-OCT-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Maladministration of the adjuvant only) and inappropriate dose of vaccine administered (Verbatim: Maladministration of the adjuvant only). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 13-NOV-2025 This patient received two doses of Shingrix on the same day. First dose was just the adjuvant portion. Second dose was properly reconstituted and given. The batch number was not provided and a request for this information has been made.
