At least half the dose leaked out while administering/butneedle used was defective and at least half of the dose leaked out, so patient did not get full dose.; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of accidental underdose in a 58-year-old female patient who received Herpes zoster (Shingrix) for prophylaxis. On 05-NOV-2025, the patient received Shingrix. On 05-NOV-2025, an unknown time after receiving Shingrix, the patient experienced accidental underdose (Verbatim: At least half the dose leaked out while administering/butneedle used was defective and at least half of the dose leaked out, so patient did not get full dose.).
