Symptom narrative

accidentally gave Kinrix to a 2-month-old patient; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate age at vaccine administration in a 9-week-old female patient who received DTPa-IPV (Kinrix) for prophylaxis. On 16-SEP-2025, the patient received Kinrix. On 16-SEP-2025, an unknown time after receiving Kinrix, the patient experienced inappropriate age at vaccine administration (Verbatim: accidentally gave Kinrix to a 2-month-old patient). The outcome of the inappropriate age at vaccine administration was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 17-NOV-2025 Reporter stated that they accidentally gave Kinrix to a 2-month-old patient. Parent of patient only wanted patient to received DTaP and IPV and Kinrix was the patient's 1st dose of each. The batch number was not provided and a request for this information has been made.
